donor Advised Fund Grant Form
Thank you for recommending a grant from your donor advised fund. Grants are made to the Levelland Wallace Theater, which will direct the gift to the designated program.
Grant recommendations must be accepted and approved by the donor advised fund sponsoring organization. My/ our expression of intent is not to create a legally enforceable obligation.
*If this is for a capitol project, the funds donated can be used to build, support, operate, and to pay debt, both principal and interest, for the project.
All gifts to the Levelland Wallace Theater are subject to and administered in accordance with the provisions and by-laws of the theater that relate to gift acceptance and administration, which include the theater’s ability to repurpose funds to a use as close as possible when the original use is no longer capable of fulfillment.
